Lost loved ones #Notforgotten

Has someone you loved died during COVID-19?

We are collating people’s experience of losing people they love during the pandemic. We want to know how it affected different groups of people in different settings and log this so they are NOT FORGOTTEN, and we can learn from your experience at such a difficult time.

Report your experience of losing someone you love during the Coronavirus (COVID-19) Pandemic #notforgotten

We are the Be Human Movement

We have set up this initiative because we believe it is important to maintain a public record of how people experienced the loss of loved one during the Coronavirus pandemic.  It will be used to increase transparency and to influence the future policy and practice of government, the NHS and local councils.  By contributing your experiences, you will help to ensure people see what happened and are not forgotten. This is for all deaths not just those who died of Covid-19 but also those related during the pandemic

If you are willing to share your experience, whether negative, positive or mixed, please take part by answering a few questions for us. You can ask someone you trust to help you complete the questionnaire.

Why we are asking
This is our way to find out what has happened during the Coronavirus (COVID-19) pandemic and how people who love those at end of their life were treated. This information will be collated and shared publicly, as well as with national and local government and the NHS.

How we will use your responses and store them
People who read your answers will not know who wrote them. You do not have to share your personal details; you can remain anonymous.
Your responses will never be shared with a third party and will be stored securely using a platform call LimeSurvey, we will keep your responses for a period of two years. Once this time period has expired, we will delete your data. The report devised from this survey will however be maintained for historical and memory purpose.
You will be able to save your response to these questions and keep it as a record of your experience for yourself.

How will the information be used?
We will use what you tell us to set up a register (a record) of the different experiences people have faced. We will discuss the results with the people responsible (in the NHS, in Local Authorities and in Central Government).
