People who receive a Personal Health Budget and or a Personal Wheelchair Budget (South East)

South East Personalised Care and Personal Health Budgets discovery Project

Who is the survey for?
It’s for all people who receive a Personal Health Budget (PHB) and or a Personal Wheelchair Budget (PWB).

What is the survey about?
We are interested in the impact and the quality of Personal Health Budgets (PHBs) and Personal Wheelchair Budgets (PWBs). We'd also like to know how you feel about the care and support you get and what impact the PHB / PWB has on you and your care and support.

What is a Personal Health Budget?

Who’s asking?
The survey is being carried out by NHS England’s South East Regional Team and a charity called 'In Control'.

Who will read my answers and what will you do with them?
The research team at In Control will independently review and analyse the answers to the survey and based on these, create a report describing what people have said. People who have contributed to the survey will not be identified in the report. The report will be used to help inform future plans for how services are delivered.

Do I have to answer the questions?
No, if you do not want to take part then that is absolutely fine. You can also choose to answer some or all of the questions. If you do answer them we will only use them in the way we have described.

You can ask someone you trust to help you answer the questions.

You can also ask for a telephone or video call to complete the survey or for a paper copy of the survey:
